Reduced Cost Blood Screenings

Reduced Cost Blood Screenings will be offered throughout the month of October 2024

To participate, a voucher must be purchased in the cashier’s office at Galion Hospital, the registration desks at Bucyrus and Ontario Hospitals, or the Bellville or Shelby Walk-In Clinics from October 1-31, 2024. Vouchers cannot be purchased at Ontario South Campus.

The following vouchers will be offered:

• HealthLink Lab Profile – $30
  (Lipids, Chems, CBC)
• PSA – $25
  (Prostate-Specific Antigen)
• HgbA1c – $30
  (Glycosylated Hemoglobin)
• TSH – $15
  (Thyroid Stimulating Hormone)
• Vitamin D, 25 – $30

Vouchers can be redeemed during the month of October 2024 at any Avita Hospital, the lab at Ontario South Campus, or the Bellville or Shelby Walk-In Clinics. Vouchers can be purchased and redeemed on the same day if testing preparation requirements are met. Vouchers are only valid until October 31^st, 2024.

PLEASE NOTE: Your physician/provider will NOT receive a copy of your lab report. Your results will be mailed directly to you.
It is YOUR responsibility to share these results with your provider.
Vouchers are not available for purchase or redemption at the Ontario Walk-In Clinic.
