Get Vaccinated!

Avita Ontario Pharmacy is offering the COVID-19 vaccine (Moderna, Pfizer-BioNTech, and Novavax) to individuals 12 years of age and older.

Request a vaccine appointment online at https://www.vaccines.gov/en/ or call 567-307-7570.

Walk-ins are welcome, but appointments are preferred.

Those scheduled for a vaccine are asked to bring a driver’s license or state issued ID. Those sick with the following symptoms must wait until they feel better to be vaccinated: fever of 100.4 or higher, cough, sore throat, muscle aches, shortness of breath, diarrhea, vomiting, etc.

Vaccines are administered at the following locations:

    • Mon – Fri: 9:00am – 7:00pm
    • Sat – Sun: 9:00am – 4:00pm

Get Tested!

Avita is offering COVID-19 testing at our Bellville and Ontario Walk-In Clinics, as well as our testing sites at Galion and Ontario Hospitals. Avita’s COVID-19 test is a polymerase chain reaction (PCR) point-of-care diagnostic test that uses a nasal sample to determine if the patient has COVID-19 infection. Results are available via Avita’s MyChart.

The Ontario Walk-In Clinic is open daily from 8:00am – 8:00pm at 600 Richland Mall in Ontario. The Bellville Walk-In Clinic is open daily from 8:00am – 8:00pm at 987 State Route 97 W in Bellville. These clinics provide urgent care services and no appointments are needed.

Our testing sites at Galion and Ontario Hospitals require a physician order and appointment. The hospital testing sites are open at the following times:

Galion
Tuesday & Thursday 8:00am – 4:30pm

Ontario
Monday, Wednesday, & Friday 8:00am – 4:30pm

You can request a self-pay rapid test for $100 without an order by calling 419-462-3437 during normal business hours for an appointment at one of the testing sites.

Get Tested at Home!

At-home COVID-19 test kits are available at Avita Ontario Outpatient Pharmacy. These at-home tests are covered by most insurance plans. Depending on your plan, you may be able to receive the test kits at no out-of-pocket, up-front costs. Other plans may require you to pay out-of-pocket and then submit your receipt for reimbursement. Purchase quantity and reimbursement limits apply. Call 567-307-7570 or visit Avita Ontario Pharmacy for more information. Pharmacy hours are Monday-Friday 8:00am – 8:00pm and Saturday-Sunday 8:30am – 5:00pm (closed daily 12:30-1:00 for lunch).

Out-of-Hospital Treatment Options for COVID-19

Oral Antiviral Treatments

Avita is offering two oral antivirals, Pfizer’s Paxlovid and Merck’s molnupiravir, for the treatment of COVID-19 in certain patients. Individuals who have a positive COVID-19 at-home or lab-confirmed test within the last 5 days, and who have mild to moderate disease with high risk of progressing to severe disease or hospitalization can be considered for treatment. Treatment is self-administered at home as an oral medication(s) taken over the course of five days. Patients must meet specific criteria to qualify for treatment. Because treatment must begin within 5 days of symptom onset, it is important to seek treatment as soon as possible after symptoms begin. Contact one of Avita’s primary care providers or walk-in clinics to see if you are eligible and discuss treatment options.
