Avita Health Foundation Now Accepting Scholarship Applications
The Avita Health Foundation is now accepting scholarship applications from college-aged students and Avita Health System employees who are pursuing a course of study in an Allied Health field during the 2025-2026 academic year. All scholarships are made possible by the Avita Health Foundation and funded by donors and the Annual Dr. Theodore Ebner 5k Run/Walk.
Applications must be received online by Friday, May 16, 2025. To date, these scholarships have provided more than $100,000 to area students and Avita employees choosing to pursue a medical career in Allied Health.Â
Knowing the value of education and the costs involved, the Avita Health Foundation, is pleased to offer these scholarships to the next generation of health care providers who will strengthen and care for our Community in the future. Scholarship recipients will be announced at the end of May.
Dr. Theodore Ebner Allied Health Scholarship
Eligibility
- The candidate must be one of the following:
- A graduating senior from a high school located within Crawford, Richland or Morrow Counties
- A student enrolled in a post-secondary institution whose permanent address is within Crawford or Richland County
- A child of an active part-time or full-time employee, who has successfully completed one year service with Avita Health System, who is a graduating high school senior or enrolled in a post-secondary institution (regardless of address).
- The candidate must be currently enrolled or accepted for attendance at an accredited vocational school, community college, or baccalaureate degree-granting institution.
- The candidate must be seeking a degree toward licensure or certification specific consideration given in the areas of Exercise Science/Physiology, Occupational Therapy, Physical Therapy, Speech Therapy, or Athletic Training.
Other Considerations
- Eight scholarships will be awarded as follows:
- Two Scholarships @ $1,000 and Six Scholarships @ $500
- The scholarship must be used by January 31, 2026 for fall or spring tuition (summer courses not included).
- The scholarships will be paid directly to the institution on behalf of the student upon verification of enrollment.
General Instructions
- Submit an essay, 200-300 words in length, discussing your goals in your specific field of study demonstrating your interest and intent on practicing in the above mentioned fields of allied health.
- Fill out the attached application and include all requested documents.
- The committee, after thorough investigation of an applicant, shall present recommendations for approval by the Avita Health Foundation Board of Directors.
- In the case of a tie between candidate applications, an interview with the Avita Health Foundation Scholarship Selection Committee may be requested.
- Deadline for submitting the application is May 16, 2025.

Eligibility
- The candidate must be an active full-time or part-time employee, who has successfully completed one year of service with Avita Health System and is currently enrolled or enrolling in a post secondary institution in the 2025-26 school year (does not include summer courses).
- The candidate must be accepted for attendance at an accredited vocational school, community college, or baccalaureate degree-granting institution.
- The candidate must be seeking a degree or certification in a healthcare-related field.
Other Considerations
- Eight scholarships will be awarded as follows:
- Two Scholarships @ $1,000 and Six Scholarships @ $500
- The scholarship must be used by January 31, 2026 for fall or spring tuition.
- The scholarship will be paid directly to the institution on behalf of the employee after enrollment is verified.

- Provide a detailed explanation (in essay format) of your goals in the healthcare field you are pursuing and how receiving this scholarship would enhance your skills to benefit Avita Health System.
- Fill out the attached application and include all requested documents.
- The committee, after thorough investigation of an applicant, shall present recommendations for approval of the Avita Health Foundation Board.
- In the case of a tie between candidate applications, an interview with the Avita Health Foundation Scholarship Selection Committee may be requested.
- Deadline for submitting the application is May 16, 2025.
