Medication Management

The Medication Management Center is a pharmacist managed anticoagulation service, or "Coumadin© Clinic".

There is relatively no waiting for your lab results. You will be given your results and instructions when you leave.

How does the MMC work?

Patients are seen by a trained pharmacist at each visit. The pharmacist will perform a fingerstick lab test, evaluate the patient and determine if any dosage adjustments are needed. Patients receive education about their Coumadin© and leave with a dosage adjustment if needed.

How do I make an appointment?

You must be referred by your physician. Individual appointments are made so waiting times are short.

What does it cost to use the MMC?

There is an office charge for each visit with the pharmacist. This service is covered by Medicare and many other insurance providers.

Do I need to register at the hospital?

Before your initial visit, you will have to visit the hospital’s registration department. After that, you will be considered a "series patient", and will not have to register again for this service, until the next calendar year.

What are the hours of the MMC?

Patients are seen by appointment only, from 8:00 a.m. to 4:00 p.m. Monday through Friday. Call 419-462-4590 for more information.
